Abstract

The role of lowering homocysteine levels in preventing strokes remains inconclusive. While findings from randomized controlled trials have been inconsistent, a large multicenter study did not demonstrate any clear advantage from such treatment. Moreover, supporting evidence from genetic research is currently limited, leaving room for further investigation into individual responses.

This report highlights a case involving a female patient who experienced multiple transient ischemic attacks and was found to carry the thermolabile TT genotype variant of the methylenetetrahydrofolate reductase (MTHFR) gene. Following initiation of targeted supplementation aimed at reducing homocysteine levels, the patient showed clinical improvement. This case supports the idea that in certain genetically predisposed individuals, homocysteine-lowering therapy may offer protective benefits.
